COVID-19 SAFETY GUIDELINES
Given the uncertainty surrounding COVID-19 outbreak, Refined Glam's top priority is the health, safety and wellbeing for our clients. The following safety guidelines are to protect the client and makeup artist. The guidelines will be used in conjunction with the state rules currently in place. These guidelines will remain in effect during COVID-19 pandemic and will be adjusted when safe to do so. Violations of any guidelines will result in appointment cancellation.
Safety Guidelines

If the client is feeling sick, the client is required to inform Refined Glam in advance. Refined Glam may use a disposable thermometer to check the temperature of each client. Any client who has a temperature above 99 F will be sent home immediately and not allowed to return until symptoms are clear.

Refined Glam maintains the right to not perform scheduled services on any clients displaying any cold or flu-like symptoms. All clients will need to come wearing personal protective equipment, such as a face mask. Refined Glam will be wearing a mask and/or face shield and disposable gloves for your protection.

Only the client who is being serviced is allowed during appointment. No extra people will be allowed.

Refined Glam will clean and disinfect workstation including chair and makeup tools/products.

Due to social-distancing barriers, conversation will be kept to a minimum, to avoid droplets contaminating any surfaces. Bathroom is NOT available.
Failure to comply with any of these guidelines may result in our refusal to perform your service, for which you will forfeit any payments made towards your appointment.
I attest that:
- I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
- I do not believe I have been exposed to someone with a suspected and/or confirmed case of COVID-19 within the last 30 days.
- I have not been diagnosed with COVID-19 and not yet cleared as non contagious by state or local public health authorities.
- I am following all CDC recommended guidelines as much as possible and limiting my exposure to COVID-19.
By filling out the form below and submitting, you agree to comply with the written instructions above and agree that you are receiving makeup services at your own risk, releasing Refined Glam from any liability relating to COVID-19. Failure to comply with these written instructions or verbal instructions from Refined Glam may result in your removal from the premises.
